INDEPTH sites are working with a modified WHO instrument to measure physical and cognitive function.  This survey instrument examines self-perceived health states, mobility, self-care, pain and discomfort, cognition, inter-personal activities, breathing, sleep and energy, affect, vision, hearing, and function assessment.
